Our General Policy

All member registration information is collected through a secure website and is treated as confidential. It is not sold, shared or disclosed to outside parties. Health Plan Partners, LLC. does not collect, receive or have access to personally identifiable health or pharmaceutical records or information.

What Information We Collect

Member information is limited to that which is necessary to provide services to enrolled members. For standard group programs, this usually is limited to name, birth date, gender, dependent status, email address, address, telephone number, and employer, association or group. For non-funded programs, the information is usually limited to name, and a coded member ID number. Health Plan Partners, LLC. does not collect, receive or have access to personally identifiable health or pharmaceutical information. We are contracted with Envision Pharmaceutical Services / Rx Options, Inc. (EPS/ROI) for administrative services necessary to provide our programs and services to members. EPS/ROI maintains strict privacy and confidentiality controls over the data it collects during the process of providing pharmaceutical services. To Whom We Disclose

All member information is maintained in strict confidence and is utilized solely for administrative functions directly related to the provision of our programs and services. We are contracted with Envision Pharmaceutical Services / Rx Options, Inc. for services necessary to provide our programs and services to members; Basic administrative data is shared with the Envision Pharmaceutical Services / Rx Options, Inc. These business associates adhere to a strict privacy and confidentiality policy.

Our Policy With Respect To Former Customers

We follow the same privacy policies and practices for former members as are described in this notice for current members.

Our Security

We will safeguard any information provided to our company. In addition, we limit the collection and use of nonpublic personal information to the minimum required to provide members with superior products or service. We restrict access to information to those employees who need to know that information to provide products or services. We maintain physical, electronic, and procedural safeguards that comply with regulatory standards to guard any nonpublic personal information. All personally identifiable data is encrypted and stored securely, and access is limited only to authorized administrative staff.
